Entry Forms
Every one who enters the video contest needs to complete an Entry Form. You can click on the links below to download the Entry Form in either Adobe pdf or Word Format. If your printing or writing is not that clear, you are welcome to get another person to fill it out the form for you.Whatever way you fill in the form, we still still need you to sign it, and have the proper signature from your parent or guardian.

If you are recording people in your video who are under 18 years of age, you must have their parent or guardian complete a Minor Release Form. A separate form must be completed for everyone other 18 years of age who appears in your video, whether or not they are indentified by name.

If you have downloaded the Word file, you are welcome to complete as much of the form as possible on the computer.
